Avery, MD Retina Today It was an exciting summer with the 2024 Olympic Games as a backdrop to our busy clinics. We were tuning in every night after clinic, watching as world records were broken, medals were snagged by unlikely underdogs, and those labeled as the greatest of all time did what they do best. What struck us most this year was the admirable perseverance of many Olympic athletes who shattered the stereotypes of age, race, and gender; battled against injuries; and had the drive to become the first to accomplish what no one else could. We could use a little of that mentality in our clinics, if we are being honest. Not because we aren’t performing at the highest level possible—we are. Rather, it’s because it often feels like we are fighting an uphill battle with some of the patient populations we serve. Some are losing vision no matter what we do. Some of those macular holes just refuse to close. And many patients with diabetes are still walking into the clinic with devastating vision loss that could have been prevented. Annual screening for diabetic retinopathy (DR) is perhaps the easiest way to nip this problem in the bud. Lee, MD, PhD Glaucoma Today Transscleral cyclophotocoagulation (TSCPC) is a laser procedure used to reduce aqueous humor production through ciliary body destruction and thereby lower IOP. With diode laser TSCPC, an 810-nm laser system transmits energy through the sclera to the ciliary body via a handpiece placed along the perilimbal conjunctiva, typically with the patient under retrobulbar or peribulbar anesthesia. Two types of TSCPC are currently in use: continuous and micropulse. This article reviews the use of continuous-wave TSCPC, which we currently find to be more reliable and effective than micropulse CPC.1,2 Traditionally, TSCPC has been used in eyes with low visual potential and elevated IOP refractory to hypotensive agents and in poor candidates for invasive glaucoma surgery. With refinement of the technique, a paradigm shift in glaucoma management has occurred: TSCPC is now performed earlier in treatment algorithms, even as primary therapy,3 and for many types of recalcitrant and advanced glaucoma. We have had excellent results with significant safety and consistency in the treatment of glaucoma that is pseudophakic, aphakic, neovascular, associated with silicone oil after vitrectomy, associated with corneal transplantation, and more.4-8 We believe that TSCPC likely represents the most minimally invasive and versatile surgical glaucoma treatment presently available. TSCPC offers several advantages. The procedure can be performed in the clinic or OR, and treatments are titratable and repeatable. The risk of infection is minimal (no incisions, sutures, etc.) when performed properly to avoid neurotrophic keratitis (see the article section on treatment areas). Kataria, OD, FAAO Modern Optometry In recent years, there has been increased excitement around ocular aesthetics, and many eye care practitioners have expanded their services to include advanced treatments for dry eye disease (DED) that have origins in aesthetics and dermatology. However, not every state allows aesthetic practice by an optometrist, despite the modality’s FDA approval for DED. Each state has specific laws that may require supervision by a medical director, such as an MD or nurse practitioner, if certain procedures are performed for aesthetic benefit.1 In some states, optometrists cannot perform intense pulsed light (IPL) therapy on the face for facial rosacea, whereas, in other states, they may perform these procedures independent of a medical director. In this article, I will offer some important points to consider, as well as information that was helpful to me on my path to opening my dry eye center. WHY IS OPTOMETRY INTERESTED IN AESTHETICS? IPL therapy targets chromophores such as oxyhemoglobin found in telangiectatic vessels and melanin located in solar lentigines. Radiofrequency, on the other hand, works by targeting water molecules and creating friction to facilitate neocollagenesis and reduce fine lines and wrinkles. This overlap of improvement in the aesthetic appearance of skin and DED symptoms with such treatments is what encompasses ocular aesthetics. Aesthetics can grow more generally from here to include facial and body procedures that are truly only for aesthetic benefit. Given our knowledge and training, optometrists are best poised to discuss adnexal and eyelid anatomy with our patients in a primary care setting.
